I have read and accept the Course Descriptions and Course Requirements (including no refund/cancellation policy) section of this web site.

I accept the tuition charge of $195 (for BRC) or $80 (for ERC) and realize that payment is due with this registration to hold my spot in the course.

I am over 18 years old. I agree to bring completed Standard Waiver Form to the first day of the course.

I am under 18 years old. I realize I must provide proof of an unrestricted driver's license, an instructional permit, or eligibility for a driver's instructional permit (DE964 or DE 964E - acheived after completion of initial part of a drivers education program). I also understand that I must bring completed Standard Waiver Form and Minor Form with parental/guardian signatures to first day of the course or I may be dismissed.

I understand that my performance will be evaluated and that I must satisfactorily complete each training phase before advancing to the next level. Should my instructor determine that I am a danger to my classmates, or myself, I will not be allowed to continue the course. I understand that this decision is for my safety and the safety of others and I will abide by the decision of my instructor. I acknowledge that if my lack of progress impedes or interferes with the class that I may not be allowed to continue the class. To receive the Motorcycle Operator Training Course Completion Card, I must demonstrate competent motorcycle operation by attaining acceptable scores on the skill evaluation and the knowledge test. Failure of the knowledge test or the on-cycle evaluation, failure to appear for any part of my scheduled class, or not satisfactorily completing a phase of the course is considered a failure for the entire course. There are no refunds for any reason, including cancellations for any reason, persons arriving late, or not attending all portions of the course, failing, or for dismissal from the course.
